Role Summary
As the pharmacy technician within the Pharmacy & Prescriptions Team at Castle Place Practice, you will:
- Contribute to the development and provision of professional pharmaceutical support and medicines optimisation services in Castle Place Practice as a primary care setting.
- Provide patient focused high quality, evidence based, cost effective and safe advice and training relating to the use of medicines and medicines management systems working with multidisciplinary teams.
- Work with the Pharmacy and Prescriptions Team to ensure repeat prescribing processes are safe and effective.
- Proactively manage the reconciliation of discharge summaries and specialist review correspondence.
- Work as part of the MDT to improve prescribing practices across the Practice and Tiverton PCN.
- Work with the support of the Clinical Pharmacists to undertake patient facing activity to proactively review patients to ensure effective medicines use including medication reviews.
- Liaise with community health services, community hospitals, outpatient clinics, care homes and patients’ usual place of residence in the community as required.
- Respond to medication queries from with patients, community pharmacists, nursing homes, carers, RDUH trust colleagues and others, with support from Clinical Pharmacists as required.
- Provide patient focused advice and training relating to the use of medicines and medicines management systems working with multidisciplinary teams.

Castle Place GP Practice has a unique and exciting opportunity for a part-time (26 hours per week) Band 5 Pharmacy Technician to join our established Prescriptions and Pharmacist team.
Castle Place Practice, which is part of the Royal Devon University Healthcare NHS Foundation Trust, is uniquely positioned to thrive in the ongoing development of integrated care in the Eastern Devon System. A friendly practice within a market town holding 14,100 patients on its list this is an exciting time to join the team.

We are located within spacious modern premises adjacent to a community hospital with a wide range of services including radiology, inpatients and urgent care. We are very much a team-based practice with a democratic approach across specialities. We pride ourselves on providing excellent care with a supportive ethos. We meet regularly throughout the day including at morning huddle and coffee.
Our multi skilled team extend to Nurses/ANPs/Clinical Pharmacists/Paramedic/Physio and being a teaching and training practice makes this a dynamic place to work. We are one of two practices within Tiverton Primary Care Network (PCN) which makes us an effective size for collaboration. Work life balance is important to us and maintaining enthusiasm for general practice through learning and development.
