Role Summary

The Associate Community Matron is a pivotal member of the integrated Health and Social care team linking with and supporting Primary care, Community Nursing and the multidisciplinary team at practice and complex care team level for patients with long term conditions.

The Associate Community Matron will work towards setting up, implementing and evaluating the Associate Community Matron role supporting patients with Long Term Conditions and complex needs. This will inform and develop the Trust’s provider service management approach to Long Term Conditions, contributing to the care of complex patients. 

The Associate Community Matron will case manage and work with patients and their carers to maximise on the benefits of the Associate Matron role in supporting their care programs, encouraging self-care and self-management approaches to long term conditions.

The Associate Community Matron will work towards developing expert clinical skills relating to long term conditions and case management. The role is pivotal in ensuring patients receive appropriate care and treatment as close to home as possible and that health and social care is patient centred and delivered to a high standard. This involves making close links with secondary care providers to facilitate appropriate and timely discharges. 

The Associate Community Matron is a pivotal member of the Primary Health Care Team and develops professional relationships with medical colleagues, other health professionals, Social Services, voluntary agencies, secondary care hospitals, community hospitals and liaison staff, to enhance the service and care pathway provided to patients who have complex needs and their families due to the presence of Long Term Conditions.

If you answered 'yes' the above then you could be just what we're looking for to join our Urgent Community Response Team (UCR) covering the Honiton, Ottery St Mary and Cranbrook areas.

The Urgent Community Response Team is an integrated multi-disciplinary community-based service comprising of Occupational Therapy, Physiotherapy, Nursing including Community Matrons and an Admiral Nurse, Co-ordination, Adult Social Care, Support Workers and Rehabilitation Support Workers. We work with our patients to manage any short-term health and/or social care needs they may have. For example, during a period of acute ill health, when their main carer is suddenly unable to provide the support the patient requires, following an injurious fall or when returning home after a hospital admission. We work closely with the teams within our Trust as well as many others including local GP practices, care providers, Hospiscare and Marie Curie. We are committed to patient-centred care and have a culture of shared learning, integrated working and promote positive risk taking.

This exciting role will give you the opportunity to further enhance and develop your clinical skills and participate in the support and training of support workers as well as identifying and exploring service development opportunities. You will have roles in triaging referrals, rapid response and case working. You are supported to undertake additional study and training to further develop in your role and are encouraged to engage with services UCR links with to build on existing or establish new working relationships. We are keen to have a strong team player join our successful team; somebody who can manage their own workload autonomously but also work closely in an integrated manner with other team members, sharing learning and supporting colleagues.

The successful candidate will have a wide range of experience working with older adults, end of life care delivery and patients with complex needs. Alongside this, the successful candidate should possess the knowledge and skills to provide a rapid assessment and safe discharge planning with the appropriate level of risk taking which comes within the Admission Avoidance and Discharge to Assess (Home First) models.

You will be joining us as we near our first anniversary of our local response to the national Ageing Well programme. This has seen Urgent Community Response teams extend service delivery to 08:00-20:00 across 7 days per week. This role will require active participation in a clinician rota that covers 08:00-20:00, including some weekend working. The current job description used as a template for this post is attached; this is subject to amendment as the UCR job descriptions are currently being reviewed and re-written. Working patterns or preferences can be discussed.

Whilst the primary base for this role will be in UCR, as a successfully integrated and flexible cluster, there may be some requirement to, at times, support or work within other teams including the Community Nursing Team. A car driver is essential. Whilst this is a full-time post, we welcome those looking for part-time hours to apply, but would request you state your preferred number of hours in your application.

Working pattern: 37.5 hours/ week
