Come and join our Community Nursing Teams in the Woodbury/Exmouth/Budleigh Cluster and help us to continue providing high quality and innovative care to the people of Exmouth in the comfort of their own homes.
The post holder will be part of an integrated Health and Social Care team consisting of Nurses, Occupational Therapists, Physiotherapists, Social workers, Urgent Community Teams and Support workers, who are responsible for delivering care to patients in their home setting, promoting independence, managing long term conditions, preventing hospital admission, supporting timely discharge and delivering end of life care.
As a Specialist in Community Nursing you will deliver high quality person centred care utilising you’re expert clinical and holistic assessment skills in order to diagnose, plan, implement and evaluate care nursing. We also require the attributes outlined in the job description and person specification to lead the Community Nursing team in managing the seven day service that can be unpredictable with day to day challenges of complexity and change.
We have a supportive and dynamic management team who truly value staff wellbeing and development. We hope you may have the Specialist Practitioner Qualification in District Nursing but if not we would support you to undertake this through the apprenticeship route.

Exmouth is a seaside town and we are surrounded by the country villages a lovely place to work. We have 7 GP practices and are supported with our Community Matron team and Urgent Community Response and Adult Social Care team.
