Role Summary

The Community Matron is a pivotal member of the Adult Health and Social Care team linking with and supporting primary care, community nursing and the multidisciplinary team at practice and community health and social care team level for patients within the community setting.

The Community Matron will set up, implement and promote the Community Matron’s role supporting patients with complex needs and Long-Term Conditions, under the Continued HealthCare (CHC) Funding Pathway.

The Community Matron will case manage and work with patients, families and their carers to maximise on the benefits of the role in supporting their care programmes, encouraging effective training implementation to enable delegated healthcare tasks to be undertaken by both registered and non-registered healthcare professional in line with the CHC care plan. The post holder will be expected to work in the community setting, in patients own homes as well as within the residential home settings. The post holder will work directly with both Primary Care and the CHC team to undertake assessments and reviews of complex patients on the CHC funded pathway, held as part of a dynamic caseload.

To support the organisation the post-holder will participate in the senior clinician on-call rota and will be remunerated in line with the Trust's Single On-call Remuneration Agreement.

The post holder will support and develop students of nursing through mentorship.

We are pleased to announce that we are looking for 3 x B7 Registered Nurses to develop as Community Matrons and join our dedicated and highly professional team of nurses and allied health professionals working in Eastern Devon. These posts will be geographically placed across, East, Mid and Exeter with the aim of improving the management of complexities with our patients who received CHC funding. This is a great opportunity for introducing innovative approaches to providing healthcare to people in their own homes and residential care settings. This role will aim to nurture our relationship and develop integrated care pathways.

As a Community Matron role, the post holder will be expected to have studied at degree level and be able to demonstrate long term conditions assessment and case management skills. The post holder will proactively manage, co-ordinate and support patients with multiple complex long-term conditions that are CHC funded and be responsible for ensuring that the delegation of care tasks are undertaken and that the providers of these tasks receive support, education and training to enable safe patient care.

The post holder will have significant levels of specialist knowledge and experience in long term conditions management, and work collaboratively with other appropriate services to ensure patients are appropriately stepped up and stepped down within the CHC funded pathways. The role will ensure that assessments and reviews are carried out, care and planned interventions under CHC funding are implemented safely and held within a dedicated caseload that is proactively managed.

This post brings great scope for integrated working, dynamic caseload management and for the post holder to demonstrate strong clinical leadership skills and develop and enhance relationships with care provider colleagues. The position will include driving to support patients within their own home and residential settings, and the ability to undertake an advanced nursing assessment will be vital, it would be a desirable should you hold the V300 Non-Medical Prescribing qualification.

Working Pattern: 37.5 hours per week
