Role Summary

The Respiratory Team at the Royal Devon University Healthcare  Foundation Trust has an exciting opportunity to recruit four enthusiastic and self-motivated trainees to gain experience in Respiratory Medicine and the management of complex respiratory diseases. This is a fixed term post (12 months) starting from August 2024.

It provides candidates with the opportunity to gain experience in aspects of respiratory medicine- interstitial lung disease, asthma, pleural/cancer/interventional. There may be the opportunity to extend fellowship roles if candidates wanted to gain experience in other areas of Respiratory Medicine.

All fellowship roles will also include elements of ward duty, audit and research and development. 

Research

The Respiratory department has well established links with NIHR and have undertaken a large number clinical trials over the years including commercial trials, and NIHR but also individual academic research.

The successful candidate would be encouraged to become involved in these exciting research programmes, writing papers and presenting at national and international conferences.  We also support the candidate to develop their own areas of research interest.

Pleural/Interventional:

This post is suited to either trainees with minimal experience or more senior trainees wishing to consolidate or develop their procedural skills, whilst continuing to gain experience in respiratory medicine. A pre-requisite for this post is that applicants should ideally have some experience of standard fibreoptic bronchoscopy and Level 1 thoracic USS. 

The service consists of the following:-

Weekly Ambulatory Pleural clinic
Busy sub-regional EBUS service (2 lists per week)
Medical Thoracoscopy service
Indwelling Pleural Catheter Service (IPCs)
Bronchoscopy Service (2 lists per week, including a regional BAL service for ILD).
Fibero-optic bronchoscopies, EBUS and medical thoracoscopies are performed in a purpose-built endoscopy unit. The unit is equipped with video-bronchoscopies and has facilities for videoing procedures and capturing digital images. We also have a bronchoscopy simulator for training purposes. 

The fellowship role will also include elements of ward duty, audit and opportunity for research and development.  

Bronchiectasis:

This post is suited to either trainees with minimal experience or more senior trainees/post CCT clinicians wishing to consolidate or develop their sub-specialty experience. The exact split of time spent focusing on bronchiectasis, cystic fibrosis and mycobacterial lung disease will be dependent on the needs and interests of the successful candidate.

The fellowship role will also include elements of ward duty, audit and opportunity for research and development.

Asthma:

The fellow would be expected to attend asthma clinics, prepare and be a key contributor of the multidisciplinary meetings and help to develop the South West severe asthma network, which would allow the completion of quality improvement work.

In total there would be 2-3 clinics per week with consultant supervision.  Contribution to inpatient specialty referrals for asthma and COPD (supervised depending on their level of competency), and development of the respiratory in reach service.

There will be the opportunity to develop guidelines and pathways for the asthma and COPD services, allowing the post holder to provide evidence of leadership and change management skills.

Time would be allocated to include the attendance of relevant lists in the endoscopy / radiology department, attendance at specialist ENT, physiotherapy clinics and Allergy and Immunology MDTs and training in lung function and airways challenge tests. 

It would be expected that the fellow would participate in the relevant BTS national audits, partake in patient recruitment to clinical trials and undertake quality improvement projects suitable for national/International presentation.

ILD:

The Exeter Interstitial Lung Disease Service is a designated Regional Specialist Centre and an anti-fibrotic prescribing centre serving the South West Peninsula, a population of ~2.5Million. This post is suitable for either trainees with minimal experience or more senior trainees wishing to consolidate their knowledge and skills.

The successful candidate will be encouraged to take an active role in audit, quality improvement and there will be an opportunity to undertake substantial research for interested candidates. There is no commitment to the medical take, unless specifically requested, or unless it is a requirement of the candidates training. 

Inpatient ward work will be minimal and will support the needs of the ILD service; general respiratory inpatient ward work is optional and for trainees with a dedicated training need.
