Role Summary
This is an exciting opportunity to join the specialist Alcohol Liaison Service as an Alcohol Liaison Assistant Practitioner. Working under the guidance of the Lead Alcohol Liaison Nurse, you will play a key role in identifying and supporting patients with alcohol-related health needs across the acute hospital setting. You will support interventions, care planning, and signposting to specialist services, helping individuals improve their health and wellbeing whilst reducing alcohol-related harm. The role is based at Bowmoor House and involves close working with hepatology teams, ward staff, community partners, and external agencies.

As part of the multi-disciplinary team, the post holder will provide a key role in providing care and support to individuals as specified in a written care or service plan, in a range of environments.
The objectives are to:
• Screening and identification of alcohol use, using Local public health and National initiatives.
• Support Registered Practitioners in their duties and contribute to the holistic care of patients as part of a Multi-Disciplinary team
• Provision of brief advice, brief intervention, motivational interviewing and goal planning.
• Develop, with the Lead Alcohol Nurse, specialist education programmes
• To be involved in audit, high level data collection, and recording on trust IT systems.
• To link in and liaise with external specialist agencies and provide professional referrals
• Attend relevant MDT, board rounds, and safeguarding meetings
• To be able to identify patient difficulties with communication and advocate where necessary
• Work autonomously on occasion and, within identified competencies, report any issues to the specialist team
Working Pattern: 17.25 hours a week, pattern to be confirmed
