Role Summary

To provide personal care & support to individuals as specified by the health or social care professional case manager. This is identified in a written care or service plan, delivered in the individual’s home or appropriate care setting.

The objectives are to support people, who need help because of age, disability or personal circumstances, end of life, to enable them to remain in their own home/care setting with as much independence as possible.

We currently have vacancies in the Urgent Community Response Exeter (Central & East) team, providing support to patients at home.  This includes following discharge from hospital, after illness or surgery and also help with end-of-life care.

As a Band 3, you will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role.

As this is a community-based position, a driving license is essential as is access to a vehicle. It is also important to note that our UCR team are lone workers within the community, and although support is always available, that isn't for everyone!

Urgent Community Response provides care across Exeter and outlying areas, and you will be part of a team providing a quality service promoting independence, dignity and support. This service aims to avoid unplanned hospital admission for people who could safely be maintained and supported at home, as well as enabling people to return to their home as quickly as possible from hospital.  The service operates on a 7-day roster system between 07:30 - 22:00 hrs, including weekends and Bank Holidays. 

It is essential that you have a kind and caring nature, are patient and able to offer a friendly smile. You must feel comfortable working on your own supported by a team of professional colleagues. 

Duties will include provision of personal care, clinical duties where delegated, monitoring the patient’s condition and working with health and social care professionals to ensure that patients are safe and well at home.  You will need great interpersonal and communication skills and be able to connect with individuals with varying levels of understanding.

You will be provided with a comprehensive induction programme which will include support to complete the National Care Certificate.

You will also need to be competent with IT as electronic recording is an essential part of the role. 

As this is a community-based position, you must have a full UK driver’s license with access to your own vehicle (with appropriate business insurance) as you will be travelling to patient’s homes to care for them. 

Whilst you will be based in Exeter there may be occasions when you could be called upon to travel to assist other Community teams.  You must demonstrate willingness and flexibility to work across multiple sites/localities as the need arises.

Urgent Community Response Support Workers are lone workers within the community, although support is always available.

Working Pattern: Hours and shifts are offered on a full or part time basis. This will include weekends, bank holidays and evenings at enhanced rates.
