Urgent Community Response Assistant Practitioner/Nursing Associate

Role Summary

We are looking to recruit highly enthusiastic and motivated qualified Assistant Practitioners  (AP’s) or trainee Assistant Practitioners (TAPs) to be part of our Urgent Care Response Team.

The successful candidates will work in accordance with the Trust framework for Assistant Practitioners  or be willing to undertake the training programme to achieve AP status. Trainee APs will receive funded training and support to undertake their studies. A clear commitment to learning is vital for those seeking the trainee AP role and will require a NVQ 3 or equivalent. You will be paid at Band 3 level until you achieve your AP qualification.

In this position you will work with the Urgent Community Response team to reduce Hospital admissions to ensure that patients receive the appropriate care in the most appropriate setting. To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making and to support patients who are in Health or/and Social Care crisis that is putting them at risk of admission.

You must be able to work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role, working within a Multi-professional team undertaking Practitioner role.

Implementing treatment and care to patients under the guidance and supervision of a registered professional.

You will need access to your own vehicle and business insurance.

Working Pattern: 37.5 hours per week, between 8am - 8pm, 7 days a week
