Role Summary

To join the other registered staff in responsibility for coordinating, delivering and maintaining a high standard of day-to-day clinical care for all patients in the Stroke Support Team case load.  

The overall job purpose of the Stroke Specialist Registered Nurse is to provide clinical expertise within the multi-disciplinary team. This includes acting as an expert practitioner, liaising, guiding and advising the Multidisciplinary Team and external agencies in the provision of optimum nursing care for stroke patients.

Please note that this job may involve driving around the whole of the Eastern patch so own car and license is a must.

An exciting and rare opportunity has arisen to join the Early Supported Discharge Team for Stroke as a Stroke Specialist Registered Nurse. This role involves working alongside the wider Multi disciplinary team to ensure a timely discharge from the acute setting to enable optimum stroke therapy with the patient's own home or another community placement.

Working Pattern: 37.5 hours per week. 08:00 to 16:00 over 7 days including alternate weekends.
