Role Summary
Provide specialist assessment, diagnosis, treatment and advice to patients and their carers. The caseload will include a wide range of acute and chronic care cases, many having complex disabilities and needs. Some services will also include terminal care cases.
Be part of a multidisciplinary team working in a community setting including rapid response.

Are you a band 5 Physiotherapist ready to progress your career? Or a band 6 Physiotherapist looking for a better work-life balance at an innovative Trust? We are a Community Health and Social Care Team in North Devon, a beautiful part of the world.
You’ll be joining a team of like-minded professionals to provide personalised care, making a huge difference to the patients we see. As a specialist Physiotherapist you’ll be providing high quality, individualised and compassionate care to our community patients, supporting their rehabilitation. We meet a variety of patients who are referred to us for Physiotherapy or Occupational Therapy input.
We provide therapy for patients on discharge from hospital, we work with patients who are at risk of being admitted to hospital with the aim to prevent the admission where appropriate. We see our patients in their own homes, in group sessions, in day care or care home environments.
There is opportunity to supervise junior team members and be involved in student education.

A typical day as a community physiotherapist
You will be based at Barnstaple Health Centre, co-located with social care and community nursing promoting a timely and successful journey for our shared patients/clients.
A typical day involves catching up with the wider team, planning and attending your community visits, as per the client’s requirements, some will be lone working, others will be joint visits either with therapy or the wider team.
MY CARE is our electronic health record system, this allows you to record live at the point of seeing the patient using your NHS mobile phone as well as on your laptop back in the office.
Your daily routine will involve regular communication with other health and social professionals and wider partners including Millbrook our equipment provider, the Independent Living Centre, Care Homes Team, GPs, the Community Response and Recovery Team and One Barnstaple (voluntary sector).
Our shared role being to support our patients/clients and their presenting needs safely within their own home/environment.
Hours and Working Pattern: A healthy work-life balance is essential, and Devon is a great place to enjoy it. This community therapy role is for 37.5 hours per week Monday to Friday with additional on-call and weekend working responsibilities. For more information, please see the Job Description.
