Role Summary
The post holder is a member of a multi-disciplinary rehabilitation team responsible for the provision of rehabilitation and nursing services in the community.
The post holder will be carry a caseload and is responsible for the assessment of rehabilitation and nursing needs, including planning, implementing and evaluation of the individuals’ nursing and rehabilitation programmes.
The post holder will have managerial responsibilities for delegating nursing components to appropriate staff including monitoring and supervision.
The post holder will be expected to work autonomously and provide support and be a resource to other team members.
The post holder will participate in and encourage all quality assurance initiatives with particular emphasis on a patient approach/implementation of essence of care.
The post holder will liaise with other professionals and managers as appropriate to ensure a high standard of rehabilitation and nursing care and plan ongoing service development.
The post holder will work as required to cover patient and service needs across the Rehabilitation, Intermediate Care and Community team.
The post holder will annually identify written personal objectives with the DN Team Leader in line with the Knowledge and Skills Framework.

We have an exciting opening for an enthusiastic and motivated Band 6 Specialist Nurse – Community Rehabilitation. This position requires experience of working with the elderly, excellent assessment skills and a commitment to undertake the Non-Medical prescribing course. You will be well supported and encouraged to continuously develop to meet the needs of yourself and the service. You will be part of a multi-disciplinary, integrated team, working mostly with the elderly frail population in their own home. You will provide in-depth assessment with the aim of identifying health deficits and then formulating a plan of collaborative treatment, including health education, to optimise their health and function and to maximise independence and quality of life and reduce the risk of falls and hospital admission.
You will liaise and work closely with the Therapists within the team, as well as Community & Specialist Nursing Colleagues, General Practitioners, Community hospitals and the acute hospital.
The Seaton, Axminster and Sidmouth Cluster has an attractive mix of rural villages and coastal towns whilst being only 30mins drive from Exeter. The Cluster is set in a landscape of outstanding natural beauty, and part of the Jurassic Coast World Heritage Site. The largest landslip in the UK provides the landscape for coastal walks, fishing harbours and beautiful beaches. Inland, the market town of Axminster provides excellent rail links to London and Exeter..
