The Bideford Community Rehabilitation team provides a service to the residents of a beautiful, rural, coastal region of Devon covering a large area of the North and West part of North Devon. There is easy access to stunning beaches and moorland making this an ideal location to enjoy work-life balance. We cover 4 GP practices and work closely with all members of the Health and Social Care team. We provide patient-centred care in patients’ homes, Nursing and Residential homes. You will require wide-ranging clinical skills & community experience to be able to work autonomously, as well as part of a team. You will need excellent communication skills, adaptability & resilience to change. There is day to day Clinical support from Clinical leads as well as a structured in-service training programme to help support and guide your development.

The ideal candidate will have a passion for Community Rehabilitation, Multi-disciplinary working and management of patients with a wide variety of presentations including complex Neurological and Orthopaedic conditions as well as dementia and falls prevention for example. The emphasis is on maximising patient independence and quality of life through assessment, rehabilitation, prevention of Hospital admission and supporting Hospital discharge.

Applicants will require confidence in problem solving, prioritisation and excellent communication skills. Our offices are shared with Community Nurses and Social Workers, allowing a high level of integrated working to the benefit of our patients.

We are committed to the continuing professional development of all our team members and offer a supportive environment with regular supervision, CPD groups, training and peer support. The successful candidate will be responsible for the supervision and development of students and support staff assigned to them. Excellent IT skills are required to coordinate the smooth running of the service and utilise the benefits of our online patient record system.

Working Pattern: Monday to Friday 8:00 to 16:00
