Role Summary

  • The completion of Health Assessments and Continuing Healthcare Tools to determine the eligibility for Continuing NHS Healthcare and Funded Nursing Care working within the National Framework.
  • To work within the target timeframes as set out within the NHS Framework.
  • To ensure that the process is implemented in a timely, efficient and transparent manner in accordance with the National Framework for Continuing Healthcare and Funded Care 2022.
  • Health Assessments, checklists and Decision Support Tools to be undertaken for clients in their own homes, residential care homes, community and other settings as directed by line manager.
  • The client group have highly complex needs and requires specialist in depth assessment of their health care needs
  • Provision of specific advice and guidance on the National Framework to the individual, relatives, independent sector, NHS and Social care staff.
  • To maintain a high and consistent standard of assessment.
  • Promote closer working relationships between the independent sector and the NHS and disseminate good practice
  • To undertake case management and case co-ordination for those within care homes or with high complex health needs.
  • To seek appropriate consent and undertake Best Interest Meetings as needed
  • Monitor and report on the quality of care delivery within nursing/care homes, liaising with regulatory bodies as necessary.

More detail about the role

The role requires nurses to use the full range of nursing and clinical skills to assess the complex healthcare needs of individuals within a variety of settings, including individuals own homes and residential homes. Completing assessments in line with the National Framework for Continuing Health Care and Funded Nursing Care. The skills you will need to successfully undertake this role are good communication and interpersonal skills, clinical assessment skills and an ability to work with individuals, families, care providers and other professionals within the health and social care sectors.

Continuing Health Care Team work Monday – Friday between 08.00 – 17.00 and is based within Ottery St Mary Hospital however we use a flexible approach to working from home and within the office. You will need to be able to travel around Exeter and occasionally out of area to undertake assessments and complete case management work.  You will need to be computer literate able to use word processing systems and typing up reports and providing good report writing.

Working Pattern: Monday to Friday 08.30 - 17.00
