More detail about the role

The Clinical Lead position for the Hospice at Home Team is a rare, exciting and fulfilling opportunity. The Clinical Lead provides expert clinical leadership for the team and service, practicing and role modelling professional excellence, and operational oversight to ensure the delivery of safe, high-quality, person-centred end-of-life care within the Sid Valley community. Working as part of a neighbourhood and multidisciplinary team approach, the post holder supports and work alongside our health and social care colleagues in the community and acute sectors for people to receive care in their preferred place of residence, maximising independence and quality of life, and quality end of life care and support.

The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08:00-16:00, 10:00-18:00 and 12:00-20:00. After 20:00 hrs, Sidmouth Hospice at Home Nurse Specialists are part of the on-call service, and respond to our patients who need specialist end of life care and carer support, and avoiding unnecessary and unwanted hospital admissions.

As the Clinical Lead Specialist, you will be required to work in core hours, Monday to Friday, to provide Specialist Clinical Leadership, and non-medical prescribing for
the team and patients. Since its beginning in January 2021, the service has enabled 95% of our patients to be cared for by their families at home as their preferred place of death.

The role combines advanced clinical practice with leadership responsibilities, spending approximately 80% of time providing direct clinical support through patient contact, complex case management, triage, clinical supervision, and audit activity, and crucially, non-medical prescribing. The remaining 20% is focused on operational and managerial duties, including safer staffing, rostering, line management, service development, and quality improvement.

As a visible and accessible clinical leader, the post holder will act as a key point of contact for complex clinical decision-making, escalation management, and professional support to the wider team. Responsibilities include undertaking holistic assessments, care planning, prescribing within scope of practice, coordinating interventions, and evaluating outcomes to ensure the highest standards of care.

The post holder will lead on clinical standards and workforce development within the Hospice at Home Team, fostering a culture of continuous learning, professional development, and evidence-based practice. They will support the education of pre- and post-registration learners, contribute to multidisciplinary working, and play an active role in service improvement projects and innovation as delegated by Nursing Leads and Senior Operational Managers.

Through effective leadership, collaboration, and communication, the post holder will ensure the Hospice at Home Service delivers its core objectives within allocated resources while promoting excellence in patient care, staff wellbeing, and service performance.
