Role Summary
Provide clinical support and care to patients and carers. This role provides support to the nursing staff and patients who will be delivering treatments closer to home supporting the virtual ward concept and the virtual ward clinical pathways and management to patients within the community setting. The post holder will support staff with the care and administration of identified treatments that can be delivered close to home and negating the reason for an inpatient stay, this is in accordance with evidence-based clinical guidelines/pathways.

This role will support the Specialist Nursing team to support patients within the day treatment units across the Community Nursing clusters of North Devon The post holder will report to the established Band 6 within the day treatment units in the North Devon area. They will also work closely with the Community Nursing service, including Community matrons also virtual wards developing the community pathway further and working on the onward care of the patients coming through the service to reduce risk of admission.
Posts agreed as part of the ambulatory care virtual ward model to offer treatments to patients closer to home.
Working Pattern: 37.5 hours per week.
