Honiton, Ottery St Mary and Sidmouth (HOSMS) Dementia Services has a unique opportunity for you to join our small supportive team of 10, including CSWs, RN’s, OT and Admiral Nurses, who work alongside our Health and Social Care MDT colleagues, Voluntary Sector and local communities and groups/businesses.

We are based at Ottery St Mary Hospital and travel out to either a person’s home for individual assessments and to deliver Cognitive Stimulation Therapy (CST) as part of their assessment, or to one of our Hubs to provide and deliver group CST and socialisation sessions for people living with dementia in each town of Honiton, Ottery St Mary and Sidmouth.

We are looking to recruit a Band 3 Support Worker who is motivated and excited to work with people living with dementia and their families.

HOSMS Dementia service is part of the wider MDT in the area specialising in the Assessment, ongoing support, Cognitive Stimulation Therapy and Meaningful Activity for people living with dementia. The aim is to keep people active, independent and included in society, to prevent unnecessary hospital admissions, reduce the chances of carer breakdown and delay the need for 24-hour residential/ nursing care.

As a band 3, you will have a level 3 Diploma/ NVQ 3 in Health and Social Care or equivalent, and ideally have experience of working with older people in a community setting.

You will have ample opportunity to develop your skills further in all aspects of Dementia care. Having the ability to be a good communicator is an important skill to have in this role. The applicant should be kind, caring and compassionate, have skills around observations of people, and being organised.

You will need to be competent with IT as electronic recording is an essential part of the role.

As this is a community-based position, a driving license is also essential, as is access to a vehicle.
