Rehabilitation Support Worker

Role Summary

Are you dynamic, motivated and have a passion for supporting independence? We have a new opportunity for a Rehabilitation Support Worker to join our large and friendly team.

Our Rehabilitation Support Workers work closely alongside our Physiotherapists and Occupational Therapists to support prescribed exercises, promote safe moving and handling skills for patients and care givers and issue equipment as directed. Applicants will require sound moving and handling skills, good problem solving, prioritisation and excellent communication. Our offices are shared with District Nurses, Social Workers and Urgent Care, allowing a high level of integrated working to the benefit of our patients.

Whilst the main base of this role will be within our Community Rehabilitation Team, you will be able to gain experience with inpatients, outpatient clinics, exercise groups, and domiciliary visits. Our caseload covers a variety of conditions from post-op rehab to long-term condition management, falls prevention and same-day equipment to keep patients safe at home. The successful applicant will be responsible for their own caseload which is supervised by our qualified Physiotherapists and Occupational Therapists.

We are committed to the continuing professional development of all our team members and offer a supportive environment with regular supervision, CPD groups, training and peer support. We also have access to a Champion’s Network that links you with therapists who are keen to share the knowledge of their special interest.

Excellent IT skills are required to coordinate the smooth running of the service due to our online patient records programme.

Working Pattern: Whilst the majority of shifts are completed 8.30 to 4.30, the successful candidate will enter the team rota to complete late shifts which support our Urgent Community Response Team (10-6pm or 12-8pm depending on service need).
