Role Summary

We have an exciting opportunity for a Rehab Support Worker within the Long Covid Service. The Long Covid service is a relatively new service with ongoing learning around Long Covid treatments and management, with scope for innovation and service development to continue in improving the service that we provide to our Long Covid patients.

This role is substantive within the Community Division with the possibility of working in an alternative service dependant on capacity, demand and funding.

This is an exciting role with lots of opportunity to increase your knowledge and skills in the area of Long Covid as well as widen your holistic rehabilitation skills, especially around respiratory, fatigue management and Long term conditions case management. You will be working closely with the Therapist and Nurse to support the patient on their rehabilitation journey, including providing education and advise to the patient and families, supporting with the Fatigue and Breathlessness virtual groups and completing face to face visits or virtual visits to meet the individuals goals.

You will be working alongside and supported by the Long Covid team consisting of a Consultant, Occupational Therapist, Nurse and Physiotherapist, as well as the wider services such as Talkworks, Community rehab teams, voluntary sector and other specialist services

Working Pattern: 37.5 hours per week. Pattern to be confirmed.
