Role Summary
To assess, provide and review nursing care, advice and information to patients and carers within the home environment or alternative care setting.
Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
To work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using their skills and knowledge as a Registered Nurse.
Maybe expected to hold a designated caseload and deputise in the absence of the Nurse Specialist (Community).

More detail about the role
The team currently provides a service from 16:00 – 08.00hrs worked over 2 shift patterns 16:00 – 00:00hrs and 22:30 – 08:00hrs. The team supports the community populations across Exeter, East and Mid Devon; we are currently based in Ottery St Mary.
We have permanent vacancies but would welcome applications for 6 month secondment to enable you to broaden your experience.
You will be able to demonstrate your enthusiasm and commitment to community nursing, and further develop your clinical skills in meeting the varied needs of patients out of hours. The post-holder will be supported by Band 6 nurses, the Team Manager and Clinical Matron. The team undertake visits in pairs, in line with the Lone Working policy.
The Out of Hours Nursing Team sits within the Health and Social Care Community Team for Honiton, Ottery St Mary and Cranbrook; the team consists of nurses and healthcare assistants. The caseload covers a broad range including end of life care, admission avoidance and support for timely hospital discharge and urgent unplanned nursing needs. The team also provide clinical support for the Urgent Community Response Support Workers between the hours of 17.00 – 22.00hrs.
Applicants will be required to commit to all shift patterns within the team and be comfortable with driving, sometimes long distances across the geography. The team have a small pool of vehicles available for any of the team to use for home visits.
We support and encourage additional learning to develop your clinical knowledge base. On appointment to the team you will be assigned a ‘buddy’ who will support you on your journey within the OOH Nursing Team.
Experience in community nursing is desirable for these posts but not essential as full training will be provided and the post holder will be supported to develop in the role. There will also be many opportunities to develop and progress your career.
