Role Summary

  • To provide clinical leadership within the ward unit.
  • To be responsible for the standards of patient care and services within the team, ensuring that nursing practice and patient services are in line with NMC Codes, statutory and Trust requirements.
  • To provide direction, supervision and control to team members in order that they provide patient care and services which are appropriate to the individual needs of the patient and the ward/unit.

Do you have an active interest in system wide approach to patient care. The Community Hospitals provide exciting opportunities for you to develop your nursing career in this area. You will work in a team which is part of the wider multi-disciplinary team aiming to ensure the right pathway for all patients to return home is met. Working as part of the community teams and the Adult Health and Social care team there is a system wide approach to all patient care.

The community hospitals provide care for patients requiring rehabilitation. The team works towards nursing and therapy led goals with medical input as required. This opportunity promotes autonomy and opportunity to linking very closely with the community teams to complete the pathway in the right environment for the patient.

The community Hospitals also provide high standards of care for people who have life limiting illness, who require supportive care towards the end of their life. The ward based team works with the community teams including the voluntary organisations to ensure the patient is provided with the right pathway of care that is individual to the patient.

Working Pattern: 37.5 hours, 07:30 until 20:00
