Role Summary
The post holder will work as part of the Seaton Hospice at Home Specialist Nursing Team, to deliver the Seaton Integrated Model for Palliative and End of Life Care for All. This service is commissioned and funded by The Seaton League of Friends but hosted and managed by the Royal Devon University Healthcare NHS Foundation Trust, as part of the Community Nursing service for Seaton.
This team provides a specialist 24 hour holistic and patient-centred palliative-care service for all patients who are registered with a Seaton GP and diagnosed as palliative, regardless of disease or complexity, guided by the individual needs and wishes of the patient and their family/carer.

Are you a dedicated nurse who always strives to give the highest standard of care to your patients? Do you have a particular interest in palliative care?
This job is a Band 6 post but a Band 5 Development post will be considered. We are seeking a motivated nurse who would like to develop their knowledge, skills and experience of working with patients with palliative care needs. You will have the opportunity to learn and gain experience through clinical reflective practice and continued professional development. You will be supported in this post by the Seaton Hospice at Home palliative care nurse specialists and the palliative care nurse team manager.
This is an exciting opportunity to be part of a professional, specialist palliative care team, who will work alongside members of the wider multidisciplinary team of health and social care colleagues both in the community and the acute care sectors.
The Seaton Hospice at Home model is based on the Sidmouth Integrated Model for Palliative and End of Life Care. This model integrates the available services within the community into a “one-team” approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service will provide continuity of care throughout the patient’s journey, enabling people to die in their preferred place, which is often their home.
The Seaton Hospice at Home Service will be managed by the Royal Devon University Healthcare NHS Foundation Trust and funded by the Seaton League of Friends.
Working Pattern: The service will provide specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00 and 12.00-20.00. After 20.00hrs an RN will be on-call to respond to our patients who need advice or a visit overnight, thus providing specialist end of life care and carer support 24/7, avoiding unnecessary and unwanted hospital admissions.
