Role Summary

Join our dynamic and friendly team working with Occupational Therapists, Physiotherapists, Nurses, Assistant Practitioners, Support Workers, Social Workers and Social Care Assessors to improve patient care by facilitating safe and timely discharge from NDDH and preventing unnecessary Hospital admissions. The majority of the role is Clinical and you will be working as an autonomous Clinical Specialist. Our pace is fast moving and our Clinical caseload is diverse and complex.

The role is based primarily in A&E and the Medical Assessment Unit but also reaches across other in/out patient areas of NDDH. There is also a Community based element to the role, visiting patients in their homes for follow up and/or as part of the Acute Hospital at Home Frailty Pathway.

You will complete comprehensive holistic assessments which will require you to work across traditional professional boundaries. You will formulate specialist management plans to ensure patients are fully managed and then discharged safely and appropriately.  

You will have the rewarding role of helping to drive forward improvements in Clinical standards within the team and have the opportunity to engage and educate the wider Healthcare team in order to improve the Care of vulnerable patients.

Due to the generic nature of the role you will need to have a wide range of experience preferably across Community and Acute Settings.

Working Pattern: 37.5 hours per week or 22.5 hours per week, between 08:00 - 20:00, 7 days a week

There is a fixed term position available for 22.5 hours per week for 12 months. For current NHS employees this post could be a secondment opportunity.

Preference will be given to staff with ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon.
