Role Summary
The Care Coordinator role is to support our existing team of Coordinators in providing help and support for our patients, GP's, Management and other members of the Primary Health Care Team. The role entails co-ordinating various work streams and providing a welcoming and friendly professional service to the general members of the public. The role is a wide and varied one that is still developing. The successful candidate will work closely with patients and clinicians to coordinate our patient's health care needs, directing them to the appropriate Service and ensuring that they have access to the most suitable care. You will be the first point of contact for the Care Homes covered by the Tiverton Primary Care Network (PCN) supporting to ensure that our patients changing health needs are addressed by considering local priorities, health inequalities and/or population health management risk stratification.

At Castle Place Practice we are looking for a Care Coordinator to work within our busy Care Coordinator team, providing support to our Patients, GP's, Management, and other members of the Primary Health Care Team. The position available is a permanent position for 23 hours per week working Monday to Friday between the hours of 8.30-6.00pm. Contracted hours to be discussed and agreed with the successful candidate.
This is an essential role, helping to coordinate various work streams and provide a welcoming and friendly professional service to general members of the public, all staff, and visitors to the Practice.
Previous experience of working within the GP Practice or other Health Care Provider would be desirable. You must be organised, self-motivated, forward thinking with excellent customer service skills. It is important that the candidate is empathetic towards patients and can remain calm under pressure and deal with patient queries whilst answering calls and dealing with GP requests, be assertive and able to multi-task.
This role is a wide and varied one that is still developing. The successful candidate will work closely with patients and clinicians to co-ordinate patients’ healthcare needs, directing them to the appropriate service ensuring that they get the most suitable care from whatever health or social care provider is appropriate.
You will also be the first point of contact for the Care Homes covered by Tiverton PCN, and you will build and maintain good working relationships with care home staff. You will be working to ensure the patients changing needs are addressed by considering local priorities, health inequalities and/or population health management risk stratification.
