Are you a motivated Registered Nurse or Allied Health Professional looking to make a real difference to patient flow and discharge planning within community services?
We are looking for an enthusiastic and experienced Registered Nurse or Allied Health Professional to join our inpatient team at Exmouth Hospital as an Onward Care Clinician. This is an exciting opportunity to play a pivotal role in coordinating patient flow, supporting timely discharges, and ensuring patients receive the right care, in the right place, at the right time within Exmouth Community Hospital Inpatient Rehabilitation Unit.
The Onward Care Clinician (Band 6) is a key clinical leadership role within the community inpatient service, responsible for coordinating patient flow, supporting safe discharge planning, and facilitating timely admissions from acute settings into community hospital beds. The post holder may be either a Registered Nurse or Allied Health Professional.

Working closely with the multidisciplinary team (MDT), the Onward Care Clinician leads daily board rounds, coordinates admissions and discharges, completes complex assessments, and acts as the primary liaison between health, social care, and community services to ensure seamless transitions of care. The role promotes a patient-centred approach, ensuring patients and families are actively involved in discharge planning and decision-making.
The post is integral to maintaining patient flow across the wider health and care system by reducing delays, supporting effective bed utilisation, and ensuring patients receive the right care in the right place at the right time. The role also provides expert clinical support to ward teams, assists with operational decision-making, and contributes to the delivery of safe, high-quality rehabilitation and intermediate care services.

This role offers the opportunity to work autonomously while being part of a supportive and experienced multidisciplinary team. You will need excellent communication and IT skills, strong clinical reasoning, and the ability to build effective relationships with patients, families, colleagues, and partner organisations.
Our diverse caseload includes patients with neurological, orthopaedic, long-term and complex conditions, including dementia. You will play a key role in supporting rehabilitation, promoting independence, and facilitating safe transitions of care through assessment, treatment planning, and specialist advice.
Collaboration is central to our service. We work closely with NHS multidisciplinary teams, Adult Social Care, and voluntary sector partners to deliver seamless, person-centred care across our local communities.
As a senior clinician, you will provide leadership, support, and supervision to Band 5 nurses and therapists, alongside nursing and therapy support workers, helping to develop the skills and confidence of the wider team.
We are looking for an autonomous practitioner with relevant postgraduate training and experience, who is motivated to contribute to service development and deliver outstanding patient care.

Working Pattern: 37.5 hours per week, Monday to Friday, 8.00 - 16.00 with occasional weekend working.
