Role Summary

Provide specialist assessment, diagnosis, treatment and advice to patients and their carers. The caseload will include a wide range of acute and chronic care cases, many having complex disabilities and needs. Some services will also include terminal care cases.

Working as a member of the Multi-disciplinary Urgent Community Response team you will undertake assessments, clinical diagnosis and treatment of patients who may require management and/or rehabilitation interventions in their own environment to prevent acute hospital admissions and to support complex hospital discharges.

Although expected to work autonomously you will not be alone. The team includes Physiotherapists, Nurses and Urgent Community Response Support Workers in addition to the Occupational Therapists, and work closely with colleagues from Frailty services, GP surgeries, Locality health hubs, Adult social care and other community services.

The qualities we are looking for in successful applicants are:

Excellent communication and time management skills.

Flexible approach to work with the ability to work across a seven day service.

Ability to work both independently and also as part of the wider multidisciplinary team.

Willingness to undertake ongoing professional development and committed to providing high quality, safe and effective care that meets the needs of patients.

Working Pattern: Flexible working pattern - 08:00 - 20:00 across 7 days
