Integrated Specialist Palliative Care Specialist (Internal Only)

Role Summary

The Sidmouth Hospice at Home Community Nursing Team deliver 24hour specialist end of life care to patients and their families during their last year of life in the Sid Valley. The successful candidate will be welcomed into our caring team, working alongside our sister charity, Sidmouth Hospice at Home, and will be based at the charity’s new building in Sidford.

As a specialist palliative care nurse, you will have on-going access and be positively encouraged to engage in your own learning and professional development, at your own pace and with supportive clinical supervision.

You will be part of the Specialist Palliative Care Team at the Royal Devon.

You will be joining a professional, specialist palliative care team, who work alongside our health and social care colleagues both in the community and the acute care sectors.

The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00 and 12.00-20.00. After 20.00hrs, as a Clinical Nurse Specialist you will be part of our on-call service to respond to our patients who need our specialist advice or a visit overnight, thus providing specialist end of life care and carer support 24/7, and avoiding unnecessary and unwanted hospital admissions. Since its beginning in January 2021, the service has enabled 95% of our patients to be cared for by their families at home as their preferred place of death.
