The Hospital Discharge Team is a newly formed multidisciplinary team bringing together Complex Discharge, Adult Social Care and Single Point of Access services. The postholder will be expected to complete comprehensive, holistic assessments, provide expert support to ward staff and resolve discharge delays to ensure patients are on the correct discharge pathways and patient discharges are achieved safely and efficiently.
Applications are invited from experienced Registered Nurses with a passion for patient advocacy, problem solving and patient flow. A background in both Community and Acute services would be an advantage.
Knowledge of health and social care funding and eligibility criteria would be beneficial but not essential.

The team is currently evolving and there is an opportunity to be involved in shaping our future.
Working Pattern: 0800-1800 variable days. Full time hours would be preferred but we can be flexible for the right candidate.
