An opportunity has arisen for an a registered nurse or pharmacist, with extensive experience within Cardiology, to join our established heart failure service, working as part of the wider cardiac support team in Barnstaple. The post holder will provide care and support to patients living with heart failure, offering advice and providing management plans to reduce symptomatic burden and improving quality of life.
The cardiac support team consists of arrhythmia care, cardiac rehabilitation nurse and heart failure nurse services, who work collaboratively with consultant cardiologists, cardiorespiratory and primary care teams to provide holistic management, support and rehabilitation to patients with various cardiac conditions. The team is well supported and championed by the consultant cardiologists, which has supported a driven and determined team to provide care of an exceptional quality.
The heart failure service provides clinic based support in the local DGH and community hospitals across the beautiful countryside of North Devon, with those patients who are unable to attend clinic offered home visits. There is also a significant amount of telephone contact to help monitor patients response to treatment changes and help avoid hospital admissions. The service has expanded to involve in-reach support and management to those admitted to NDDH, and the post holder will be expected to contribute to this area of care.
We would encourage both nurses and pharmacists, who have a passion for cardiology to apply for this post. We are predominantly a nurse led service, but have experienced the real value of a multi-disciplinary team, with pharmacists and other professionals providing a crucial contribution to patient care and enriching the knowledge / experience of our teams. We are keen to ensure our team / service continues to benefit from several disciplines, but will judge suitable candidates based on prior knowledge and experience, without favouring any particular profession.
Applicants who wish to work in our small, but friendly team of professionals, should be highly motivated and an enthusiastic team player, with an empathy and passion for caring for patients living with cardiac conditions. You should have excellent interpersonal skills and a professional manner; managing your workload on a day to day basis in response to the demands of the service.

If you do not meet the full person profile, but are keen to develop, it is possible that we would consider a developmental post at Band 6. An agreement and plan would be made together to achieve progress towards Band 7.
