We are seeking enthusiastic and motivated candidates to join our friendly Urgent Community Response (UCR) team that are based in Crediton, Okehampton and Moretonhampstead.

We are looking for an enthusiastic person, with excellent IT skills and the ability to work on their own initiative to join our friendly Health & Social Care Team covering the Crediton, Moretonhampstead and Okehampton Locality. Knowledge of Care First 6 and EPIC is an advantage but training can be provided.

The role involves regular communication with clients, families, clinicians (from a wide range of disciplines) and the voluntary sector requiring excellent customer care skills. You will be expected to monitor the inbox, answer phones, ensuring all referrals and communications are dealt with in a timely manner. Additionally, you will accept referrals for care and book visits on the rota for the UCR team working closely with the acute hospital and clinical staff to help prevent hospital admission and a safe supported discharge. You should be a good team player, supportive, flexible and willing to offer help to your colleagues.

The Post holder will be expected to work in a team as well as on their own, using their own Initiative.

We welcome applications from candidates who can demonstrate good interpersonal and administration skills along with strong prioritisation skills and a positive can-do attitude. This is the ideal role for a person who wants to make a difference and enjoys change in a fast-paced working environment.

Working Pattern: You will be required to work your hours over 5 days, covering a 7-day service, 365 days a year. You will work weekends and bank holidays, on a rota basis, resulting in enhanced hourly payments. Some flexibility in hours we can offer.
