Role Summary
- Responsible for the referral and coordination of urgent care response services – coordinate detailed referrals from Health & Social Care professionals, communicate and answer queries from all disciplines.
- Responsible for the coordination of the Urgent Community Response Support workers using the EPIC system to produce rotas.
- To support the Community Health and Social Care Team members in the utilisation of a range of “case finding tools”. These tools will help identify people at risk of loss of independence, potential risk of unnecessary admission to hospital or longer term care.
- To be a central co-ordination point for the local multi-disciplinary activity, including the facilitation of the “core group” function. The post-holder will do this by co-ordination of regular meetings, maintenance and active progressing of active cases with all team members.
- To be responsible for a high quality customer service function in recording contact information, supporting the initial prioritisation of contacts, subsequent feedback and on-going liaison with referrers and relevant others.
- To maintain IT based information systems and take a key responsibility for the production of key performance data. High levels of typing skills and IT skills involving extensive experience of Microsoft Office including Work, Excel and Outlook is required.

We are looking to recruit a multi-skilled health and social care co-ordinator to support the daily running and administration of our integrated community health and social care services provided to patients within Honiton, Ottery St Mary, Cranbrook and the surrounding areas. This is a fantastic opportunity to be part of a successful team delivering a high quality service to all.
This post involves close working with a multitude of staff from our varied and integrated community teams including Adult Social Care, Urgent Community Response, Community Rehabilitation and Community Nursing. Within our multi-skilled post-holders, we are looking for someone to learn the skills to be able to provide cover and support across the co-ordination team but with a particular focus on our adult social care processes.
The role includes receiving and recording accurate information from and providing timely updates to other colleagues including acute and community hospital staff, GPs and cluster management as well as patients, their relatives and carers plus external organisations and local care providers. The post-holder will be required to take and co-ordinate incoming and outgoing referrals, facilitate joint agency meetings, monitor caseloads and waiting lists and handle incoming enquiries. This role will require strong organisational and communication skills including an excellent telephone manner. The post-holder will also be able to demonstrate and develop skills in producing and sharing reports and escalating to team members where patients are requiring assessment or review.
This is a really exciting time to be joining our cluster. You will be joining a well established co-ordination function and there will be the opportunity to actively engage in service review and development, as well as learn many elements of both health and social care functions.
This role will involve some weekend working. Being a car driver is not essential for this role. The base for this role is Honiton hospital; some working from home could be considered once the role is well understood.
