Frailty Advanced Clinical Practitioner/ Trainee ACP

Role Summary

The post holder will and in accordance with the Trust Framework for Advanced Practice:

  • Work autonomously as an Advanced Clinical Practitioner, across traditional boundaries, within the Frailty Team and provide a high level of expertise to ensure patient-centred clinical care.
  • Exercise advanced clinical expertise, levels of judgement, discretion and decision making whilst undertaking the skills of assessment, examination to provide a diagnosis and formulate timely treatment plans within an agreed scope of practice
  • Evaluate investigations and revise treatment plans according to patient need and assess impact and outcome.
  • Assist in the safe referral and discharge of patients with undifferentiated and undiagnosed presentations in any areas of working including Same Day Emergency Care (SDEC) Unit, Medical Assessment Unit (MAU), Emergency Department, Acute Frailty Unit, Virtual Ward, and the Community.
  • Provide expert professional advice to patients, carers and colleagues
  • Monitor and lead improvements to standards of care through; supervision of practice, clinical audit, implementation of evidence-based practice, teaching and support of colleagues and the provision of professional leadership.
  • Lead the implementation of new evidence-based practice and contribute to the development of the evidence through research, audit and collaboration with clinical or academic staff.
  • Contribute to clinical governance by leading quality improvement, audit and research projects as part of the multidisciplinary service-wide performance and quality agenda.
  • Role model the Trusts values and behaviours, demonstrating a person-centred approach to service delivery and development whilst maintaining a professional portfolio that supports their scope of practice.

We are looking to recruit a highly enthusiastic and motivated qualified Advanced Clinical Practitioner (ACP) or trainee ACP to be part of our new Frailty Team here in North Devon. The successful candidate will work in accordance with the Trust frame work for Advanced Practice or be willing to undertake the training programme to achieve ACP status.  

The ACP role will be integral in supporting the Frailty Lead with the setting up, provision and delivery of acute and holistic care of frail older people within our new Virtual Ward.  You will work across the Emergency Department, Medical Assessment Unit, Same Day Emergency Care Unit, on the wards and in the community to identify suitable patients for the virtual ward and will lead the assessment, evaluation and management of these patients.

We encourage applications from Nursing and Allied Health Professionals who are willing to work generically across traditional professional boundaries to provide holistic best practice care for frail older people in the virtual ward setting.

The virtual frailty ward seeks to manage individuals living with frailty who have an acute medical condition that would usually necessitate an admission to an acute hospital bed. With the support provided by the virtual ward team, however, the individual will be supported to remain in their own home receiving a high level of clinical care, diagnostic and therapeutic interventions to manage the acute episode.

As we move towards a new era of integrated care and working collaboratively across organisational boundaries, the need for dynamic, focused teamwork is vital and the ACP will be an integral member of the new team.  The ACP will be key in building relationships across the acute and community settings and will be provided with a wealth of learning opportunities.

Trainee ACPs will receive funded training, support and protected non-clinical time to undertake their studies.  The ACP course is a master’s level qualification so a clear commitment to learning is vital for those seeking the trainee ACP role. You will be paid at Band 7 level until you achieve your ACP qualification.

As you will be working in the community as well as the acute hospital you will need to hold a valid driver’s licence and have access to your own transport.

Working Pattern: This role is 37.5 hours per week and will encompass a Monday – Friday, 9am – 5pm working pattern. For the right candidate we would consider 30 hours per week.
