Role Summary

The Falls Assistant Practitioner is a new, innovative pilot role, progressing the Enhanced Healthcare in Care Homes work. The role will be a pivotal member of the Adult Health and Social Care team linking with and supporting the multidisciplinary team including care workers.

The Falls Assistant Practitioner will work closely with the Falls Prevention Clinical Practitioner by providing Falls Prevention and Awareness training to care workers. The post holder will also be establishing links and liaising between care homes and other aspects of the health and social care community within Exeter, Mid and East Devon. The post holder will participate in audit and research to measure clinical effectiveness and user satisfaction.

Whilst the post holder will not hold a clinical caseload, they will be expected to provide support directly to individual people as well as care providers. Please see Job Description for further detail.

We are looking for a dynamic Assistant Practitioner to join our dedicated and highly professional Falls Prevention team working in Exeter, Mid and East Devon. This is a great opportunity to introduce innovative approaches to providing Falls Prevention and Awareness training and support to the health and social care community, whilst working closely with the Falls Prevention Clinical Practitioner.

The post holder will work within Exeter, Mid and East Devon to provide Falls Prevention and Awareness training to care homes and colleagues in: Primary care, Community Nursing, Allied Health Professionals, Social Services and voluntary agencies.

The post holder should have good IT skills including, Excel, Word, PowerPoint and MyCare, as data collection is an essential part of this role.

This post brings great scope for integrated working, and the post holder will be able to demonstrate their passion for Falls Prevention and their ability to enhance relationships with care provider colleagues. This is a community-based post and will include driving to support patients/residents within their own care settings and also participate in wider team activities within the Trust both in person and through MS Teams events.

This is a fixed term contract for 12 months. For current NHS Employees this post could be a secondment opportunity
