Clinical Lead Posts:
Clinical Leads roles focus on the case management of the patients within the service, ensuring the right care, for the right time and with the right clinician. In addition, the role has an enabling focus on the knowledge, skills, competence and confidence of the teams by working alongside the teams and the patient. This role is perfect for those leaders that want to maintain direct clinical contact with the patient whilst also be given the opportunity to drive clinical change through excellence.
Operational Lead Posts:
Operational Lead post are an integral part of the leadership team across teams within localities. They support the clinical teams through information management and curiosity. The post holder will be driving performance and quality improvement in order to deliver a high standard of care whilst also focusing on Quality Improvement through change. This role is perfect for those leaders that want to maintain clinical and professional skills but are interested in system working and quality improvement initiatives.
Highly Specialist Therapist:
The Highly Specialist Therapist is open to Occupational Therapist and Physiotherapist. This is a new and exciting role that give the ability to be 100% focused on clinical activity. A strong focus on our long-term condition patients working in partnership with the Community Matrons and Response Practitioners to ensure patients have a seamless transition through their care journey and their needs are known and met through their leadership and working with others.
Urgent Community Response (UCR), Intermediate Care (IC) and Proactive Care (PC):
Following a review in relation to the National framework, UCR will focus on admission avoidance within the 48-hour timeframe.
Intermediate Care is a pathway to describe patients rehabilitation journey following a crisis and/or admission to hospital.
Working Hours: 37.5 Hours
