Role Summary
Are you seeking a new challenge working in a patient facing role in Community Services?
We have a position available for an experienced pharmacist to join our established Community Services Pharmacy Team. We are a team of skilled, patient-focused clinical pharmacists and pharmacy technicians who provide a clinical service to community hospital adult inpatient units and a home-visiting medicines optimisation service to a referred caseload of patients with complex medication needs. Exmouth Community Hospital is the base site for this role. The post holder will enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews as part of a wider multi-disciplinary team.
All Trust staff use a shared electronic prescribing and medical record (EPIC) which strengthens integrated care across acute and community services.
Role responsibilities include:
- Providing medicines advice to patients and carers, health and social care professionals and skilled non-registered staff working collaboratively to resolve complex medication queries.
- Medicines reconciliation on admission and discharge and ensuring clear onward communication to our primary care colleagues.
- Undertaking clinical medication reviews and making evidence-based clinical recommendations to ensure high quality, patient-centred care.
- Empowering patients to manage their medication safely, with support for developing confidence and capability in self-administration.
- Multidisciplinary collaboration including medical, advanced clinical practitioners, nurses, therapists, primary care clinicians, community pharmacy, mental health teams, care providers and social care staff professionals.
- Providing a high quality, safe and patient focused clinical pharmacist service to the Community Care Group; including community hospital inpatient units, outpatient clinics, ambulatory clinics and patients’ home settings to meet professional, Care Group and Trust requirements.
- Deliver high quality personalised patient care in line with legislation, national and local guidance and specifications.
- Optimise clinical outcomes, improve medicines safety and deliver best value from medicines within community services.
- To work towards a non-medical prescribing qualification, qualify and work as an independent prescriber within a defined patient caseload.
- To participate in the wider pharmacy service and its development in response to patient care needs.
- To contribute to the development of medicines management and optimisation throughout the patient care pathway.
- To provide supervision training and mentoring of staff to include post-graduate and undergraduate pharmacist placements.
- To support the wider community services pharmacy team with the delivery of training to other members of the multidisciplinary team on prescribing and medicines related topics.

We require a self-motivated and flexible pharmacist who is able to work both independently and effectively as part of a team. You will be joining an exceptional team working together to help our patients to regain independence with their medication regimes and stay healthy when they need it most.
This pharmacist will provide a competent and efficient pharmaceutical service to hospital inpatient units and patients at home to meet the changing operational demands of the service. Ideally you will have completed, or be on your way towards finishing a clinical diploma and have experience in a hospital pharmacy setting. We will consider applications from experienced pharmacists from any sector.
We are looking for a candidate who:
- demonstrates excellent communication and problem-solving skills
- shows initiative and adaptability in a dynamic healthcare environment
- will work with the wider pharmacy team to support safe and timely discharge of patients and manage a caseload of referred patients requiring an intervention at home.
- is flexible, organised and committed to delivering patient-centred care
- can prioritise workload effectively in response to changing patient needs
This position sits within an experienced, ambitious and motivated pharmacy team. The post-holder will receive appropriate training with plenty of scope to develop in the role, and enjoy a high level of job satisfaction. There is the expectation to qualify and use independent prescribing skills to facilitate excellent patient focused care while promoting medicines safety, admission avoidance and discharge facilitation.
Working Pattern: 22.5 hours per week
