Community Services Pharmacist

Role Summary

  • Providing medicines advice to patients and carers, health and social care professionals and skilled non-registered staff working collaboratively to resolve complex medication queries. 
  • Medicines reconciliation on admission and discharge and clinical medication reviews
  • Making evidence-based clinical interventions and recommendations to prescribers to provide patient-centred care. 
  • Supporting patients in managing their medication safely, including facilitating self-administration.
  • Work towards a non-medical prescribing qualification
  • Collaborating with a multidisciplinary team (including medical and ACP prescribers, nurses, physiotherapists, occupational therapists, primary care, community pharmacy, mental health teams, care providers and social care staff professionals)
  • Provide a high quality, safe and patient focused clinical pharmacist service to the Community Care Group; including community hospital inpatient units, outpatient clinics, ambulatory clinics and patients’ home settings including nursing homes to meet professional, Care Group and Trust requirements.
  • Deliver high quality personalised patient care in line with legislation, national and local guidance and specifications. 
  • Optimise clinical outcomes, improve medicines safety and deliver best value from medicines within community services.
  • To qualify and work as an independent prescriber within a defined patient caseload.
  • To participate in the wider pharmacy service and its development in response to patient care needs.
  • To contribute to the development of medicines management and optimisation throughout the patient care pathway.
  • To provide supervision training and mentoring of junior staff to include post-graduate and undergraduate pharmacist placements.

 

To support the wider community services pharmacy team with the delivery of training to other members of the multidisciplinary team on prescribing and medicines related topics.

Are you seeking a new challenge working in a patient facing role?

We are looking for an experienced pharmacist to join our established Community Services Pharmacy Team, a team of skilled and patient-focused clinical pharmacists and medicines management technicians. We provide a clinical pharmacy service to community hospital adult inpatients as well as a medicines optimisation service to patients with complex medication needs in their own homes. This role will be in and around Exmouth and will be based primarily in Exmouth Community Hospital. You will work with colleagues to enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews.  

 Employees of the Trust benefit from using a shared electronic prescribing and medication administration record (EPIC) which supports integrated care across acute and community services.

 We are looking for candidates who:

  • will work with pharmacy technicians to support safe and timely discharge of patients and manage a caseload of referred patients requiring an intervention at home.

You will work in an experienced, ambitious and motivated pharmacy team and be offered appropriate training with plenty of scope to develop in the role, and enjoy a high level of job satisfaction. There is the expectation to qualify and use independent prescribing skills to facilitate excellent patient focused care while promoting medicines safety, admission avoidance and discharge facilitation.

 We are looking for a self-motivated and flexible pharmacist who is able to work both independently and effectively as part of a team. You will be joining an exceptional team working together to help our patients to regain independence with their medication regimes and stay healthy when they need it most.  

The candidate will provide a competent and efficient pharmaceutical service to hospital inpatients and patients at home to meet the changing operational demands of the service. Ideally you will have completed, or be on your way towards finishing your clinical diploma and have experience in a hospital pharmacy setting. We will also consider applications from experienced pharmacists from any sector.

Hours: 22.5 hours per week

Salary: £47,810 to £54,710 pro rata

The salary advertised for this Band 7 position reflects the Agenda for Change 2025/6 rates, inclusive of the 3.6% pay award which has been implemented.
