Role Summary
Due to the Management of Change and the expansion of the community function within the Single Point of Access (SPoA) we have an opportunity within Exeter for another Response Practitioner (RP) role.
Urgent and Emergency Care is a fast-developing area of practice and this is an exciting time to be involved in these new ways of working in this Trust. Over the past couple of years, the UCR Team has been developing its skill set with a variety of different professions and roles to support a wider range of patients in the community so that where possible, a hospital admission can be avoided. The RP role and Advanced Clinical Practitioner (ACP) roles are part of this change, providing a more clinically skilled workforce in the community.

We have also been establishing closer working relationships and processes with the acute front door services, the Acute Hospital at Home (AHAH), and the Hospital Outreach Team (HOT) team to ensure there is senior medical oversight for the community patients who require it, and re-assurance for the teams in supporting these patients at home. The SPoA plays an important role in guiding referrals to the right department or team to provide the care and medical support they require, and having an experienced community presence in a room of acute clinicians has shown how when we work together, we can coordinate the required support in the community, therefore negating the need for admission.
The RP role will be twofold;
- understanding what services are available in the community; communicating and coordinating this with our acute colleagues, while providing a link with the UCR Teams
- maintaining and developing clinical skills working with the UCR team and community patients.
Working Pattern: 37.5 hours per wek
