Role Summary
Community Nursing is entering a period of exciting transformation and innovation to protect the future service we provide for our patients and sustainability of the NHS. In Line with the NHS Long Term Plan and People Plan 2021, Community Nursing Services will be expanding and evolving to meet the ever-changing needs and demands of the NHS. Community Nursing provides holistic patient centred care for patients who are housebound, usually with complex, chronic and long-term conditions. Nursing for these patients required sound clinical knowledge and competence to provide sound independent judgement.
The work Community Nurses do is hugely varied so your portfolio of clinical skills can grow in complex and advanced nursing competence. Our core work includes Palliative care, supporting people at home with complex and long-term conditions, administering medications, advanced tissue viability and vascular access.
We work closely with the wider community MDT to promote admission avoidance and empowering patients to stay at home to receive their nursing care. We work very closely with our GP, Hospice, Physio and Occupational Therapy, Social Work, Voluntary and Urgent Care Services.
Community Nursing prides itself on providing a great work life balance with business hours working, in a friendly and supportive environment. Your wellbeing and development are important to us. We can offer learning and development opportunities and flexible working to meet your personal and development needs.
