Role Summary

  • To assess, provide and review nursing care, advice and information to patients and carers within the home environment or alternative care setting.
  • Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
  • To work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using their skills and knowledge as a Registered Nurse.
  • Maybe expected to hold a designated caseload and deputise in the absence of the Nurse Specialist (Community).

As the new manager for Tiverton Community Nursing, having started in September 2022 – I continually strive for a service that delivers nursing excellence in our community. Since commencing in my role, many positive changes have been made to the service. I have an ethos of open feedback, building a better workplace together, and developing my team. I believe in a work place that is fair and open. We have a wide range of skill mix within the team, which benefits our patients and colleagues immensely. Due to the positive changes within the team, some former community nursing staff have now returned to our team!

You will be part of a dedicated team, delivering a wide range of clinical interventions. Candidates will need to have excellent communication and assessment skills and will be responsible for the assessment, planning, implementation, and evaluation of personalised patient centred care in partnership with our patients and those important to them.

Clinical skills for the role include; Catheterisation, Continence promotion, Warfarin monitoring, Diabetes management, Venepuncture, IV, Wound Care, Lower Limb Care inclusive of compression bandaging, Hickman/ PICC Line and Drain management, in addition to all aspects of Palliative and End of life care.  ** All training for these skills is available through the position if required. We would accept application from newly qualified to experienced clinicians. **

The Community Nursing Team is skill mixed with registrants and non-registrants. As well as working within the community nursing team, you will have opportunity to be part of the Integrated Health and Social Care team to provide multidisciplinary working, that will also enhance care delivery to the patients.

You will be provided with regular supervision and support with ongoing professional development as identified in your annual appraisal.

Candidates will need to own their own car, with business insurance as the role involves driving to visit patients in their own homes. 

Working Pattern:  Part time and full time positions available. A full time candidate would work 5 out of 7 days a week, 0900-17:00, weekdays, weekends and bank holidays. However, hours are negotiable for the right candidate, so please contact to discuss and we welcome informal visits.
