Role Summary
The Community Matron will hold a caseload of patients with Long Care health needs. They will be responsible for managing the patient's conditions with the Multidisciplinary teams. The Community Matron will work closely with the Community Nursing Teams, providing clinical oversight where required. This role covers Exminster and Ide Lane Community Nursing Team patients.

This is an exciting time to be part of Royal Devon University Healthcare NHS Foundation Trust introducing innovative approaches to providing Healthcare to people in their own homes, preventing admissions to Hospital and being a pivotal member of a Health and Social Care Multidisciplinary team engaging with our partners in secondary care and facilitating patient discharge from the acute sector .
In the Community Matron role, the post holder will be expected to have studied at Degree level and able to demonstrate long term conditions assessment and management skills and hold the V300 prescribing qualification.
The post holder will have significant levels of specialist knowledge and experience, for example in Long term conditions management, care of the elderly and Chronic Obstructive Pulmonary Disease. This will be evidenced through short courses and/or continuous professional development demonstrated in the application and at interview. The post holder will be expected to have achieved advanced clinical academic modules related to Long term condition management and the independent Supplementary prescriber module, BV300.
The expectation of the role is to ensure that patients receive appropriate care and treatment as close to home as possible and that Health and Social Care is patient centred and delivered to a high standard. This involves making close links with secondary care providers to facilitate pulling patients as appropriate through the system. The post holder will be expected to develop professional relationships with Medical colleagues, other Health Professionals, Social Services, voluntary agencies, secondary care Hospitals, Community Hospitals and liaison staff, to enhance the service and care pathway provided to patients who have complex needs and their families.
The post holder will be expected to independently explore all avenues to support patients with complex needs and chronic disease including forming effective communication networks across organisations to achieve patient focussed goals.
The successful candidate would be able to demonstrate knowledge and understanding of current issues relating Primary Care, Community Nursing, Social care and integration.
You will need to own and drive your own car as travelling between patient visits and across other venues is part of this role. There is a mileage allowance which is calculated to reimburse your expenses.
Working Pattern: 30 hours per week, 08:30 - 16:30, four days per week to be agreed
