​We are looking for a Community Matron to join our supportive, dedicated and highly professional team of Nurses and Allied Health Professionals working in the Hospital Discharge Team, in the Royal Devon University (RDUH) Hospital. This is a 6-month fixed term opportunity for introducing innovative approaches to providing palliative discharge and advance care planning support to patients with a life-limiting illness.

  • To promote and deliver advanced Clinical skills to assess discharge requirements and future care plans for Royal Devon and Exeter Hospital patients with life-limiting illnesses.
  • To promote dignity, encompassing a person-centred approach to positively enhance the lives of patients with complex life limiting illnesses.
  • To review, evaluate and develop the discharge pathway for patients with palliative and end of life care needs.

We are an integrated Health and Social care team based on the acute site providing Clinical assessment and triage for patients face to face to prevent unnecessary Hospital admissions or to facilitate appropriate discharges. We also work to support patients through the end stages of their life. This role involves using a patient centric approach to completing Nursing assessments and formulating discharge plans.

In this Community Matron role, the post holder will be expected to have studied at Degree level and have the ability to work as a confident autonomous practitioner. You will need high level Clinical assessment and problem-solving skills to manage complex patients who require assessment of needs prior to discharge. This post brings great scope for integrated working, and the post holder will be able to demonstrate strong Clinical leadership skills and develop and enhance relationships with Care provider colleagues.

You will work in partnership within an integrated team including medical colleagues in the RDUH, Primary Care, Community Nursing, Allied Health Professionals, Social Services, voluntary agencies and any other relevant services. You will proactively co-ordinate and support the Hospital discharge process for patients with palliative diagnosis or long-term conditions. You will have significant levels of knowledge and experience in long term conditions and community palliative care, and work collaboratively with other services to ensure patients are appropriately supported within the discharge pathways. The role will ensure that assessment and review are carried out, referrals for care and planned interventions are implemented safely, and that appropriate, safe and effective discharge is proactively managed.

The post holder will be expected to work in the acute setting as part of the Hospital Discharge Team (HDT), liaising regularly and effectively with relevant community services. The post holder will work directly with Clinicians in the RDE, particularly in the Emergency Department (ED) and Acute Medical Unit (AMU), alongside the Hospital Specialist Palliative Care Team where appropriate.

The HDT Community Matron will promote the role within HDT, supporting patients with complex needs, life-limiting conditions and a palliative diagnosis. The post holder will demonstrate palliative care knowledge and skills in the capacity of primary discharge link between Royal Devon and community services, advising on decision-making around palliative care issues and access to other Health and Social Care services on discharge, to support the needs of patients and those close to them.

Working Pattern: 37.5 hours per week, over 5 days.

This is a fixed term contract for 6 months. For current NHS employees this post could be a secondment opportunity.

Preference will be given to staff with ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon.
