Role Summary
- To support the Community Health and Social Care Team members in the utilisation of a range of “case finding tools”. These tools will help identify people at risk of loss of independence, potential risk of unnecessary admission to hospital or longer-term care.
- To be a central co-ordination point for the local multi-disciplinary activity, including the facilitation of the “core group” function. The post-holder will do this by co-ordination of regular meetings, maintenance and active progressing of active cases with all team members.
- To be responsible for a high-quality customer service function in recording contact information, supporting the initial prioritisation of contacts, subsequent feedback and on-going liaison with referrers and relevant others.
- To maintain IT based information systems and take a key responsibility for the production of key performance data.

About the Team
The Single Point of Access team who are located within the main Wonford Hospital site operate a seven-day service that provide a single point of contact for patients, families, carers and associated people, the team facilitate and co-ordinate the safe and timely discharge from Hospital for our patients in partnership with other multi-disciplinary colleagues. The team are based within the Community Division which is spread across the Devon area ranging from Okehampton (Mid Devon) across the City of Exeter to the Seaside town of Seaton (East Devon).
About the opportunity
An exciting opportunity has arisen for full-time discharge co-ordinator to work with the team on a 12-month FIXED term or SECONDMENT basis. The position will be 37.5 hours over 4 days. The discharge coordinator is an essential role within the team and you will form part of a supportive and close working group of professionals working in partnership with other multi-disciplinary colleagues across the Eastern Area. You will be working with the team to increase patient flow and optimise patient experience surrounding transfers of care. As a team of therapists and nurses we provide face to face triage and clinical assessment of patients to support their onward care needs. This is an exciting role that has a great deal of variety within the working day.
About You
We are looking for an enthusiastic, motivated and caring person with excellent communication skills to co-ordinate and support the team, an ability to work in a fast-paced environment and problem-solve with be essential, the role will include documenting actions on meetings, co-ordinating the team’s workload and liaising with a variety of health & social care colleagues across the Eastern area. You will be required to answer the telephone and respond to e-mail enquiries in a timely manner and knowledge or understanding of the health and social care environment would be desirable.
Working Pattern: 37.5 over 4 days
