Devon Integrated Care System has been awarded Stroke Catalyst Funding by NHSE to test new models of care across the Stroke Rehabilitation Pathways across Devon.  This project aims to improve the interface between the community stroke teams and the short-term care services, delivered by health and social care teams, to optimise packages of care and maximise patient outcomes and experience. 

The post will be hosted by Royal Devon University Healthcare NHS Foundation Trust, but will work alongside the community stroke and social care teams across Devon.  The post-holder will work with stakeholders, to oversee the development and delivery of a regional stroke competency-based training programme, for carers working within the short-term care services.  You will work with care providers to complete a training needs analysis in relation to care delivery with stroke patients and maximising opportunities for rehabilitation during care visits.  You will have a proven working knowledge and understanding of stroke rehabilitation, as well delivering single-handed and technology enabled care to optimise packages of care, patient outcomes and patient experience.  Part of the role will involve supervising the locality Stroke Rehabilitation Practitioners who will be working directly with the stroke patients and the care services to inform and implement care plans, and embed the learning from the education programme, to maximise patient recovery and independence.

Innovation is central to the stroke and neuro-rehabilitation services at the Royal Devon, demonstrated by the implementation of several new initiatives and the development of non-traditional roles in recent years including a Research and Innovation Lead, Consultant AHPs, ACP and FCP roles.

Working Pattern: Full and part-time working will be considered and we have a flexible working policy that we are happy to discuss to establish suitable options for you and the department. Remote/home working is also an option for a proportion of this role. A healthy work-life balance is essential, and Devon is a great place to enjoy it.
