- Visit patients on the ward to plan for their follow up needs.
- Post discharge follow up appointments both face to face and via telephone.
- Carrying out of functional testing for patients
- Provision of group exercise sessions
- Facilitation of remote rehab programmes

The Cardiac Rehabilitation team based at North Devon District Hospital in Barnstaple, are looking for an experienced Nurse or Physiotherapist to join their team. The service provides follow up care and rehabilitation to patients who have had a cardiac event or procedure, and those with a confirmed diagnosis of heart failure. Care is delivered both in acute and community settings, with team members needing to be able to work both autonomously and as part of a team.
We are seeking candidates who have worked in a Cardiology setting and have a good understanding of Cardiovascular conditions and the NICE guidance underpinning the delivery of care in this speciality. Experience of Cardiac Rehabilitation would be an advantage; however, this is not essential, and training can be offered to those who are passionate about taking on this role and developing the skills required.
The post holder will be expected to work closely with colleagues on the Cardiology Ward, the wider Cardiac Support Services Specialist team, Consultant Cardiologists and Therapy services.

Working Pattern: 37.5 hours per week, consideration may be given to part-time hours for the right candidate.
