CNS Personalised Cancer Care and Support (Internal Only)

Role Summary

To work as a Clinical Nurse Specialist (CNS) focusing solely on the personalised care agenda, supporting the planning and delivery of care for for patients with cancer.  

They will have an oversight of the following workstreams, working with clinical teams to maintain compliance:

Holistic Needs assessment (HNAs)

Treatment summaries

GP Cancer Care reviews

Health and Well-being events

The post holder will support the effective rollout of personalised care interventions to ensure that the individual needs of people living with and beyond cancer in North Devon are met by tailored support and services. This will include care companion, community hub work and the Lifestyle & Wellbeing Fair.

The post holder will support and work closely with the Fern Centre Manager and Clinical Matron for Oncology & Haematology in managing the Living With and Beyond Cancer Programme of work across the Trust.

Maintain close working and regular communication with clinical nurse specialists, MDTs and relevant key stakeholders being aware of specific targets, understand any issues or blocks which may result in targets not being met and report to the Clinical Matron

Link in with colleagues across the peninsula to maintain good relationships and to aid share learning.

To work with the Clinical nurse Specialists, medical and nursing teams to be the link for the personalised care agenda. 

To develop and sustain partnership working with individual groups, communities and agencies. Facilitate the planning and delivery of care programmes to address patient needs and develop/improve the service.

Act as a champion, promoting the awareness of the Living With and Beyond Cancer agenda across North Devon, with the aim of increasing clinical engagement and achieving specific outcomes.

KEY RESULT AREAS

Working Pattern: 4 days a week (Monday - Friday)
