About you

We are looking to appoint into Band 2 and Band 3 level roles for UCR (see salary details below).

As a Band 2, you may not have formal experience in care, but you are able to demonstrate a passion for delivering excellent patient care with the support and guidance of your colleagues. You will quickly become a valued member of the team through our comprehensive training and induction programme and our commitment to learning and development will satisfy even the most ambitious! You will be on a pathway to be working at a Band 3 level. 


As a Band 3, you will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role.

As this is a community-based position, a driving license is essential as is access to a vehicle. It is also important to note that our UCR team are lone workers within the community, and although support is always available, that isn't for everyone! Our Urgent Community Response (UCR) team form an essential and well-respected part of our healthcare teams across a number of dynamic community settings.

As part of the UCR team you will be community-based and the primary focus of your role is to enable a person to stay safe and well in their own home. You and the team will provide a short-term intervention to clinically review, and offer care and advice for patients when they are discharged from hospital. You also support a person during a period of ill health, and aim to avoid a hospital admission if a person’s care and medical needs can be met safely in their own home, including specialist care and support at home for end of life patients.

Therefore, as part of our UCR team you will receive extra training and support to develop the specialist skills required for this role. UCR Practice Educators are available to support with education, training and competency sign off, colleague development and career pathway support, and provide a link between teams for wider communication.

The wider team encompass a variety of roles and professions including; Nurses, Community Matrons, Paramedics, Physiotherapists, Occupational Therapists, Social Workers, Social Care Assessors, Coordinators, Support Workers and Therapy Assistants.
