Role Summary

What does the role involve? 

You will be part of a friendly and experienced multi-disciplinary team based at Exeter Community Hospital. You will provide excellent patient-centred care in; outpatient clinics, patients homes, nursing and residential homes and within a leisure centre setting. 

You will be providing rehabilitation to patients with a wide range of conditions, there will be a focus on providing post trauma and orthopaedic care, as well as falls prevention work through exercise programmes and classes. Therefore being Postural Stability Instructor trained would be desirable. You will assist with the management of long-term conditions, and empower patients to self-manage their condition to maintain independence. You will hold your own caseload and be responsible for completing initial holistic assessments for these patients and developing subsequent goal-focused treatment plans. You will be supported through the supervision of one of our senior clinicians. 

You will also work within Urgent Community Response (UCR). The UCR function covers 8am-8pm across 7 day working and provides a 2 hour response to admission avoidance patients within the community, as well as continuing with the discharge to assess pathway. This post holder would be required to work on a rota basis within these hours.

We are seeking a Band 4 Assistant Practitioner to join the Exeter Community Rehabilitation Team in the Central and East locality. A full time vacancy for a permanent post is available, however part time employment for 30 hours and above can be considered. 

What qualifications do I require? 

  • As a minimum we require someone with a foundation degree (Level 5 Qualification) which is relevant to this job role. 
  • Or you may have a higher level qualification which is relevant to this job role. 

In both scenarios we would be able to support you to achieve the relevant Band 4 skills and competencies once you are in post. For example within; strength and balance assessments, exercise prescription and falls prevention.

Overall we require a person with good communication skills who can work both single handed and as part of a team. They will need to access patient homes within the community and at times lone work without direct supervision. 

Working Pattern: Usually Mon-Fri 08:30-4:30.  However you will assist the Urgent Care Response team approximately two shifts per month (shift times usually 8-4pm or 12 – 8pm. The ability to work between the hours of 8am and 8pm and weekends is therefore essential.
