Role Summary

The role of the Assistant Practitioner is to support the Nursing and Therapy practitioners by undertaking delegated aspects of their role for which they will have been trained and deemed competent. The Assistant Practitioner on completion of training always continues to act under the guidance of a registered practitioner.

The Assistant Practitioner does not require daily supervision and should be able to take responsibility for their work within pathways of care and that of others within a broad framework laid down by and as directed by the supervisor of practice.

We have an exciting opportunity for a Band 4 Assistant Practitioner to join our team in the Exeter South & West Cluster. We would consider applications from those already qualified or those willing to undertake a development post with further training/qualification.

The urgent community response team (UCR) provide a 7 day a service, 8am – 8pm covering the areas of Broadclyst, Pinhoe, Topsham, Exminster, Alphington, St Thomas and Starcross

The team facilitate early discharges from hospital and prevention of avoidable admissions. The post holder will work closely with the multi-professional team, including physiotherapists, Occupational Therapists, nurses, Pharmacy technician, support workers and links with social care colleagues.

The post holder should be highly motivated to support with holistic assessments, including risk assessments and formulation of patient centered care plans. without direct supervision and frequent lone working. You will need excellent communication and IT skills as well as adaptability & resilience to change. Community experience is desirable.

There is a structured induction programme, in-service training and supervision.  The post holder should hold a Foundation Degree in a relevant field and have experience in a health and social care setting, supporting patients’ independence with their activities of daily living. 

Working Pattern: The post holder will also support a 7day working rota on a 4 - week working pattern
