Role Summary

This is a community based job managed by Acute medical services. Due to the nature of the job please only consider this role if you have access to a car, a driving license and appropriate insurance. Whilst we do our best to plan visits so that distance driven is minimal , we are a team that responds to the acute service need and rapid discharge of patients so this may not always be possible.

Do you have a keen interest in Stroke Rehabilitation within the patient's home? Do you have the skills or would like to enhance your skills to be able to provide support to the registered nurses within the team and provide guidance and leadership to band 3 colleagues?

An opportunity has arisen for an experienced Band 4 Assistant Practitioner for Nursing within the Stroke Support Team. The successful applicant will work within a larger Multi disciplinary team with a focus on intensive stroke rehabilitation at home. 

The post holder will at times be working as an autonomous practitioner. Supervision will be provided when needed however there will be times of lobe working  and supervision of shift therefore the confidence to respond to a change in situation and emergency response is a must. 

The organisation is digital and therefore an ability to utilise technology is essential. You will continuously develop enhanced clinical skills and will work collaboratively with colleagues to ensure a holistic approach to all patient's interactions.

Working Pattern: 7 day service 0730-2100
