Role Summary
Would you like to be part of our dedicated team of a multi-disciplinary team who help people to stay safe and independent in their own homes or recover from injury and illness in our local community? We are looking for a Band 4 AP to cover maternity leave for a period of 12 months to join our team of Occupational Therapists, Physiotherapists and Nurses who make up the Central & East Exeter Urgent Community Response Team.
The UCR team provide an urgent response to health and/or social care needs whether they are at risk of an admission to hospital or to facilitate discharge home from hospital.
You will be visiting patients within their own home environment to assist with assessments and carry out treatment, with a goal-orientated focus, within your competencies or as delegated by registered staff. The aims of treatment are to maximise an individual’s independence, safety and quality of life using a strength-based approach. Rehabilitation tasks may include mobility and transfer practice, drink and meal preparation, personal care activities, exercise programs, practicing use of equipment, clinical supervision of band 2 and 3 Support Workers within the team, and providing community support follow-up on discharge from hospital as requested.

The role of the Assistant Practitioner is to support the Nursing and Therapy practitioners by undertaking delegated aspects of their role for which they will have been trained and deemed competent. The Assistant Practitioner on completion of training always continues to act under the guidance of a registered practitioner.
The Assistant Practitioner does not require daily supervision and should be able to take responsibility for their work within pathways of care and that of others within a broad framework laid down by and as directed by the supervisor of practice.
The post holder will need to hold a Foundation Degree in Healthcare Practice, NVQ 3 in Health and Social Care or equivalent experience. We require a person with good communication skills who can work independently and as part of a team.
Clinical supervision will be provided by a qualified Occupational Therapist/Physiotherapist/ or Nurse and we actively encourage peer support and offer learning and development opportunities.
Working Pattern: 37.5 hours per week. Variable shift patterns between the days and hours of : Monday-Sunday 8am-8pm
