Role Summary
- To undertake a clinical lead role planning, coordinating, delivering and evaluating the clinical service provided to meet complex discharge planning and admission avoidance
- To provide ongoing clinical and professional leadership, be responsible for the clinical standards and ensure the team meets the requirements of national standards and evidence-based practice and to maintain good documentation
- To work collaboratively with other team leads within the Trust to develop the service and deliver a high-quality service to patients
- To demonstrate continuing professional development that reflects individual needs, the needs of the team and the needs of the organisation
- To work with the teams as appropriate in avoiding admissions or facilitating complex discharges taking on own case load as and when needed
- Support social care colleagues with complex discharges by taking the lead with complex health/social care issues contacting wards/doctors/psychiatry in the RD&E or CCTs/community hospitals/teams and then signposting care managers to enable the discharge to progress
- Respond to management in times of escalation ensuring that appropriate action is taken and work load is prioritised

We are pleased to advertise a new full time, substantive B7 role which will build on the senior clinical presence and experience within the Single Point of Access (SPOA) Team.
This role will be managed by the community-based Professional Practice Team and will provide a unique link and community perspective in supporting Pathway 1 referrals.
The innovative post will be key in improving collaborative working and communication between services, and provide training and support for all clinical staff in triaging and decision making to help to improve resilience, knowledge and skills to streamline discharges and improve flow between acute and community services.
The post holder will be able to challenge the progression of complex discharges, working with all parties to source the right solution for the patient, and ensure documentation and communication channels are clear and effectively employed.
Working Pattern: 37.5 hours per week
